What Is a Normal Blood Pressure, by Age?
A doctor explains why blood pressure targets barely change with age, what the numbers mean, and why the old rule of adding your age no longer applies.

The short version
- For almost all adults, whatever their age, a healthy blood pressure sits around 120/80 mmHg or below, and 140/90 mmHg or above measured in clinic is treated as high.
- Blood pressure does rise with age in most populations, but that is a description of what happens, not a standard to aim for. The old idea of adding your age to 100 is obsolete.
- Home readings run lower than clinic readings, so the home equivalent of the 140/90 mmHg threshold is about 135/85 mmHg.

There is no separate healthy blood pressure for each decade of life. For essentially every adult, a reading around 120/80 mmHg or below is ideal, and a clinic reading that stays at or above 140/90 mmHg is treated as high: whether you are 35 or 75.
What does change with age is what actually happens to people. Arteries stiffen, the top number drifts upward, and by the seventies a majority of adults in most countries meet the definition of hypertension. That is a population trend, not a target, and the old rule of thumb about adding your age to 100 has been abandoned because it quietly labeled dangerous readings as fine.
What the numbers mean#
The top figure, systolic, is the pressure while the heart contracts. The bottom figure, diastolic, is the pressure between beats. Both are measured in millimeters of mercury.

Home and ambulatory readings sit roughly 5 mmHg lower than clinic ones, which is why the thresholds differ. Guidelines vary between countries, the United States uses 130/80 mmHg as its cut-off, so read your result against the standard your own clinician uses.
Why age does not get its own target#
The reasoning is simple: risk tracks the number, not the birthday. A systolic of 160 mmHg carries a similar burden of stroke and kidney risk at 70 as it does at 50. Trials treating older adults to lower targets found fewer cardiovascular events, not more harm, provided the person was not frail.
Where age genuinely changes the conversation is in frailty. In someone who is unsteady, falls, or feels lightheaded standing up, pushing the number down aggressively can cause more harm than the pressure itself. That is an individual judgement a doctor makes with you, not a general licence for higher readings.
Getting a reading you can trust#
Sit still for five minutes first, feet flat, back supported, arm resting at heart height. No caffeine, cigarettes or exercise in the previous half hour. An empty bladder matters more than people expect. Use a cuff that fits. A cuff too small for the arm reads falsely high, which is a common cause of a wrong label.
Take two readings a minute apart, morning and evening, for about a week, and average them while ignoring the first day. One high reading in a pharmacy proves nothing.
When one high reading is not the answer#
A single elevated number in a clinic is a reason to measure properly, not a diagnosis. Anxiety, pain, a rushed walk to the appointment and a full bladder can each add 10 mmHg or more. Diagnosis is normally confirmed with home or 24-hour ambulatory monitoring precisely because of this.
